CONSENT TO RISK:
I have now taken my FREE trial lessons and want to sign up for membership under the L.T.S.I. I understand that there may be some significant risks in learning Tae Kwon Do. I acknowledge that I / my child must always be responsible for safeguarding our own wellbeing and will therefore never attempt any practices or techniques that I do not fully understand.
I confirm that I must always tell the Instructor of any illness or other conditions that may affect the training or wellbeing of myself or any other person, and that currently there are no reasons why I cannot learn Tae Kwon Do.
I agree to comply with all of the Rules and Regulations of the L.T.S.I. if I am accepted as a member and, as a condition of acceptance, I agree not to hold the Lions Tae Kwon Do School Independent or any of their Instructors or students liable for any injury that I may sustain whilst practicing the Art.
Declaration:
In submitting this form to the L.T.S.I., I confirm that all the information is current and correct at the time of signing up for classes.
Furthermore, I also understand that all information will be stored in the L.T.S.I. database and will be viewed by the group secretary for administrative use only.
Student / Parent notice:
Please ensure you have filled in this form correctly, as any mistakes made will result in an admin. charge being applied for corrections.
Please Note: All members must sign and return the consent to risk form found in the beginner pack to the Instructor before starting training.
Parent / Student confirmation:
I have read this document and agree that all the information I have provided is correct. By signing up to this school, I understand and accept the risks involved in learning the art and will not hold L.T.S.I. or any of their Instructors responsible should an injury occur.
